It may not help directly, but coincidentally this is the second report in a week claiming that OMV has 'switched' to 127.0.0.1

My understanding is that 127.0.0.1 is the IP loopback address. Valid, and probably always present.
In my experience yes and yes. And only on the loopback (lo) interface. It should never appear on a "real" interface.
I have never used Open Media Vault, but I suspect some common misunderstanding of how it should be configured and used.
Me either. I suspect it's broken in someway and its UI is adding to the confusion.
