I tried using my Raspberry Pi 5 with only heat sinks without a case and suspended in free air. Temperature was around 50 with rise to 60 and also erratic processing behavior when using MyPaint, a heavy processing app.
I placed a $4 five volt constant speed fan on top of Pi 5 board, plugged into 5v pin and ground. Temperature reduced to 35 at idle and 45 under load.
Two weeks now, using MyPaint with no issues.
The noise of the constant fan is totally unobjectionable to me.
